- 1 yr Bank Statement ( Certification from Bank optional)
- Address Proof letter from company (ID card xerox optional - if have good)

Sign the application
Pay the Fee (cash only)
Get the Fee Payment Receipt
10.05 - sent to B section (wait in queue in chairs)
10. 20 - Goto Verification Officer
Verifies the originals and application
10.25 - Completed B section and wait for C section call.
10.35 - C section ask you sign the old passport cancellation form
Puts "cancelled" seal on the passport and xerox copies submitted
Gives back the old passport and formalities complete.
10.40 - Goto exit counter and give the duplicate copy of payment receipt
Counter gives you the actual payment receipt
Provide the Service Feedback in Touch Screen display
10.45 - All process finished and out of PSK with big satisfaction :)
